1. Agreement to Terms
By accessing coresupportconnection.com or any services provided by Core Support Connection, LLC (“CSC”), you agree to these Terms. If you do not agree, do not use the site or our services.
2. Our Services
CSC is a private-pay home care and support agency serving individuals with disabilities and aging adults in Adams, Brown, Clermont, and Highland Counties, Ohio. Services include in-home personal care, transportation, community integration support, and care coordination. CSC does not participate in Medicaid or Medicare.
3. Service Agreements
Home care services are governed by a separate written Service Agreement. These Terms apply to use of our website and digital platforms and do not supersede the Service Agreement. CSC reserves the right to refuse, modify, or discontinue services in accordance with applicable law.
4. Digital Portals
CSC operates a Staff Portal and Family Portal. By accessing these portals, all users agree to: use the portal only for authorized purposes; maintain confidentiality of login credentials; comply with HIPAA requirements for any Protected Health Information accessed; and not share access with unauthorized individuals.
5. Payments & Fees
CSC’s income-based sliding scale fee structure is disclosed during the intake and consultation process. All fees are due as specified in the Service Agreement. CSC reserves the right to modify rates with advance written notice.
6. Acceptable Use
You agree not to use our site or portals to submit false or fraudulent information, attempt unauthorized access, transmit harmful or unlawful content, or violate any federal, Ohio state, or local law.
7. Intellectual Property
Content, design, logos, and documentation — including the Legacy Binder system and Warm Handoff Model — are the intellectual property of Core Support Connection, LLC. You may not reproduce or distribute without express written permission.
8. Disclaimer of Warranties
THE SITE IS PROVIDED “AS IS” WITHOUT WARRANTIES OF ANY KIND. NOTHING ON THIS SITE CONSTITUTES MEDICAL ADVICE. Always consult qualified healthcare professionals for medical decisions.
9. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY OHIO LAW, CSC WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, OR CONSEQUENTIAL DAMAGES. CSC’s total liability will not exceed amounts paid for services in the three months preceding a claim.
10. Governing Law
These Terms are governed by Ohio law. Disputes will be resolved in the courts of Brown County, Ohio, or the Southern District of Ohio federal court.
